You may download HSC forms as needed, but please note that because we use COLORED paper to better serve you, using all white paper may slow down the processing of your forms.

Initial paperwork
HSC PACKETS FOR ARE DUE IN THE OFFICE NO LATER THAN THE FIRST FRIDAY IN AUGUST TO AVOID A LATE FEE. This also allows your assigned reviewer time to help you make adjustments if any are needed. The earlier you submit your packets, the easier it is for her to serve you.
HSC High School Course Credit Request Form (green) - one per subject
HSC Course Contract Evaluation Form (pink) – to be used in addition to the green form when the course does not use a traditional text, or is a course of your own design
HSC High School Course Plan (yellow) – should include all subjects for a given year
HSC Credits Earned Worksheet (white)
HSC Four Year High School Plan of Study form (white) – fill out each year to keep on track
Periodic forms
HSC Quarterly Progress Report Form (lavender) - filled out quarterly to show work completed – Please do not submit reports late. If you are waiting on a grade from an outside instructor, note this on the report card, but still turn in the report card and progress report. Otherwise we will assume no work has been done in the quarter.
Due dates* for Quarterly Report Cards, and Progress Reports are as follows for the 2011-2012 school year (subject to change-please check back)
1st Quarter – Monday, November 14, 2011
2nd Quarter – Monday, February 6, 2012
3rd Quarter – Monday, April 2, 2012 April 9th, 2012 REVISED
4th Quarter – Monday, June 4, 2012 (If a Senior, all grades must be turned in by Wednesday, May 30 – please alert your outside teachers!)
*Please note Report Cards (even if incomplete) and Progress Reports not submitted by the due dates, are subject to a $25 late fee per student.
To avoid the late fee, submit Progress Reports and a Report Card by the deadline, and follow up with updated grades as you receive them.
